Planning your visit to East Valley Ophthalmology

Your eye exam appointment will include a consultation explaining your diagnosis and treatment options. However, a complex medical history or treatment plan may require an additional appointment and evaluation to provide treatment or eye surgery on another day. Occasionally, treatment for a minor issue can be performed on the same day as the consultation.

Once your appointment has been scheduled, please follow the steps below:

  1. Download and complete the New Patient Forms.
  2. Transfer your medical records from your existing doctor to East Valley Ophthalmology. This can be done by completing the Records Release Form.
  3. Take the following items to your first appointment:
    • Current medications
    • List of questions to ask your doctor

For a successful appointment, please bring:

  • Completed paperwork
  • Insurance Card
  • Photo ID
  • Current medications
  • Medical records and lab results

Accepted Health Insurance Plans

Co-payments & Billing

Most insurance plans require a copayment to be paid at the time of service. After your appointment, your insurance will be processed, and you will be billed for any services not covered by your insurance. Please call your insurance provider directly if you are still determining whether a particular service is covered. If you have questions about a bill, please contact the phone number listed on the billing statement.
